   The head coach is responsible for ensuring that this registration/roster is completed.  All players are responsible for being knowledgeable of and aware of the rules as established by the Shelbyville Parks & Recreation Department.

   To complete your registration:

   1. Complete all required information on this form
   2. After submitting, you will be redirected to the league payment system. Refer to the individual sports league pages for specific payment amounts, deadlines, and accepted methods.

   Registration will not be finalized until both the completed registration/roster and full payment have been received.

   Please note: accommodations for special assistance can be requested by emailing parks@cityofshelbyvillein.com for any programs or events no sooner than two weeks prior to the event date. All requests will be considered to ensure an inclusive and accessible experience for all participants.

    By signing this team registration form, each coach acknowledges their understanding of all associated rules and regulations.  They also acknowledge that participation in Shelbyville Parks and Recreation programs constitutes acknowledgement and assumption of all inherent risks and release and hold harmless the City of Shelbyville, the Shelbyville Parks and Recreation Department, and any sponsors, associations, or employees related to any program or activity from any liability for injuries, claims, or damages that may occur during my participation in such events, programs, or activities. This release extends to my heirs, estate, executors, administrators, assignees, and all members of my family.
